I get my 67 masters at Napa when i do these conversions and they havnt been  that expensive. Just ask for one for power brakes non disc.   They do have the adapters too if you want to run pre-made lines, they also carry rolls of the awesome copper-steel alloy automotive line which doesnt rust if you are fabricating  For the rear the easiest is a new line from front outlet of master to a flare union with the line going to rear of car. Plug the junction block with flare plug in the outlet for the rear brakes. Then new line from the rear master outlet to junction block. That's it.
