I think it used a ground strap. 
http://www.dantesparts.com/fuelaccessories.html shows a picture.
But you can ground the unit by running a wire from the outlet tube 
to the unibody if you don't want to buy that. 

Not sure about your 2nd question. Near the filler tube I'd guess.
