1. is the bolt hitting the steering box or is it  the cam washer on the bolt?
 
2.  if the bolt is hitting it, then yes you must at least loosen the bolts that hold the steering
     box to remove it,   the adjustment cams will work which ever way they end up installed
 
3. was thinking that there were just 3 bolts , but may be 4
