Michael,
I go through Parish-Heacock in TN...I believe they are in Chattanooga.  The 
carrier is Great American.  The car must be in a garage and my policy is 
limited to 3000 per year.  No other restrictions and I believe they are 
flexible on the mileage.  I had no need to go above 3000 miles.  Great 
people to work with, I would highly recommend them.
