For 220 you have two hots and a common. Each hot supplies 110v. If you want to be smart, use the fourth wire for an earth ground, otherwise use the two wires for the common.

OK, thanks for the help guys, i just went overkill on my 220 extension
cord project to eliminate all doubt.Figured in the future I might need
to run something way bigger off it, like a huge welder.
  i got some monster ends rated at 50 amps and I bought 28 feet of 6
  gauge cord.
   man they are not  giving that 6 GA. away for free!! the most
   expensive extension cord i ever bought!!
   Anyway my question is:
    The cord has 4 strands in it , as i understand that is common now.
     WHAT should I do with the fourth strand?
   somebody  said to tie  2 strands together  so in effect u would have
   two wires of 6 ga. each and one of twice that.
  Now will that work?
  And if it would work , which should u use the doubled wire for
  -ground , hot  or neg.  Or would it matter?
  I want to do this right as this is going to be about a 115 dollar
  extension cord by the time I am done with it.
