Most of the time it's just being sent through someone's web-based mail with any easy to guess password. I have been removing these folks as it happens, and sending the following to those people:

Someone was sending spam to the Forward Look list through your email
account. This often happens if you have an easy-to-guess email
password, such as a name or common word. I suggest changing your email
password as soon as possible, and to use a mixture of uppercase and
lowercase characters, and at least one number.
Once you change your password, please shoot me an email and I will read
you to the list as soon as possible. Sorry for any inconvenience this
may cause you.

I haven't seen any advertising. So, I agree with Adam. Someone has a virus on their machine that is causing all the spam that you see.
