2X on all of the posts.  Bottom line is it can only be one (or more) of 5 things and all are easy to check except brake adjustment, low fluid, leaking master cyl, leaking wheel cyl. or brake adjustment. If the seals are bad in the master cyl., the pedal will slowly 'fade" to the floor board when constant pressure is applied. obviously the leaks will show up as wet areas. you may want to check the brake lines also. if you're losing fluid (low Master cyl.) you may have a cracked rubber brake hose or a very small rust hole in the break line.  Good luck with your investigation.
