Autozone has them(2227 & 2228). Rockauto.com lists them as well.

I didn't expect to have a problem finding motor mounts for my 64 
Plymouth with a 318 poly but so far all I have found are expensive and 
on EBay. 

I have one broken mount and would like to replace the pair. It would be 
appreciated if anyone knows of a source. 

Is there another one that can be modified to work on the poly? 

Another surprise, I tried to buy a 4001 head light at Napa and Checker, 
not listed! I haven't ordered yet but I found them on Ebay.
