From the 65 Dodge and Plymouth hemi Superstock Authenticity Guide by Jim Schild, page 20, the fan had 4 rivets laid out like in Nick's photo.
I agree with you though, in alot of the pictures/diagrams you see the 5 rivet model with the five rivets in the shape of the '5' on a six-sided die.
