Something is wrong with this info . My guess is the spacers felll out of the sure grip unit which BTW is a very common problem of the tiny roll pin breaks with the clutch type. Without the spacers the axles are too short (about 1/8 to 3/16") The cure is simple with two very easy possiblilties.
