For something that small, you could cut out the rusted area and stitch weld some flat sheet metal in. Floor's arent a bad place to learn the technique, as they're not too visible.

Another option, If there is a rustbusters franchised business in your area, you could sand it down, and have them spray both sides with motlen zinc, sand it smooth on both sides and and not have to worry about that spot rusting again. It spot galvanizes the metal that it touches to prevent further corrosion. http://www.rustbusters.com/locations.html

I have a friend who had the smaller holes filled around his rear wheel wells about 7 years ago left it sanded bare for about 5 years before painting and didnt even get any surface rust around it.

Do I just cut out the rusted area?  If so, do I use a rectangular or
circular shape?  Should I use a pattern for removal so that I can use
the same pattern ( a bit larger I'd guess ) for the replacement metal?

Are there professional welder people that can come to your house to
weld in the new pieces?  I'll probably have some frame rail connectors
to weld in place as well ...

I don't want to start cutting stuff until I have some semblance of a plan ...
