DSA-140-B no formal CCD to DSA.
DSA-140-A, then a CCD is required.'
I do not speak for DSA. If this falls into the category of being a DSA-B. I believe no CCD is required to be submitted to DSA for review and approval.
However, if this falls under the category of being a DSA-140-A, then it would require a CCD be submitted. Anything that is falls under the Structural Safety,
Fire Life Safety or, Access Compliance, then a CCD is required.

Kevin, we are having issue with a discrepancy for the torque testing torque spec on the antenna mounting hardware
vs what is listed in the Construction Drawing.
Page S-2 Detail 7 for Maywood, S-2 Detail 8 for Corning HS and S-2 Detail 9 for Woodson all call for M12 mounting
hardware however the appropriate mounting kit for pipe 2 3/8”uses M8 mounting hardware. We are asking for the adjustment to be made on all 3 drawings for Mid-Pacific to sign off as torque compete. They will observe the M8 hardware torque spec at this time
but will only offer sign off once the CD’s have been updated. Please review the clip from the CDs’. I have also attached the manufacturers installation manual for reference of “not to exceed” 14NM or 10 ft lbs.
Thank you for your assistance and hope to resolve this issue as soon as possible.

I hope this finds you doing well.
I know you don’t have the DSA approved CCD-4 yet so schedule that monopole footing pour. That would only effect one day for me.
I now have a finalized concrete pour Calendar schedule for us to work around.
I have very large Concrete pours that will occupy my time on just those days as far as any other concrete pour like yours.
10/7/23 - tomorrow Saturday
These dates would only affect the one day footing pour and are at 4:00 AM each day. I can do all the other required inspections for
Woodson Antenna project on the days I do not have the mega concrete pours and after the pour the same day.
Once we do that Woodson pour, there will be no time crunching for me..

Once we have the DSA approval for the new changes, and you schedule the work accordingly, please be aware I do have
to be on site as does the Testing Lab when you schedule the casting of that Monopole footing.
I know you will want to do it ASAP as do I.
Therefore, I hope to avoid any conflict with other projects that I have with concrete being cast. They are huge
in comparison and also require my being present. These days are already calendared and hopefully we can work around them, just for your one pour.
As it stands today, and always subject to adjustment:
I have a 100 yard pour, scheduled on, 10/10/23.
I have a 400 yard pour on, 10/13/23.
I have a 100 yard pour on, 10/17/23.
If there are any changes and we have not cast the Woodson Footing, I’ll up date you.
I trust there won’t be a conflict.
This is just an FYI and I trust I will be able to cover what I need too.
Kevin, these drawings look good, please move forward with submission to DSA for approval.

Not knowing the length of the bolts pictured, which are headed bolts, I would have to believe these bolts are not
related to the anchor bolts shown at Detail 6/S-2. Those bolts for the monopole footing at Woodson will need to be much longer.
The footing is 30” plus deep and those bolts will have to extend down to within 3” of the bottom of the footing
above the soil and project up out of the footing enough to provide full nut threads attachment, plus adding the thickness of the base plate (2”) and allow also for 3” of grouting.
They could be as much as 40” plus in length. And greater in diameter at 1-1/4”.
There are two (2), what what appear to be all thread rods, length unknown, inside the monopole sitting the past
several months at the site. Their length should be listed on the Testing Labs in plant bolt report.
If you want to know why we grout under base plates for these poles, outside light poles, etc., that can explained
at another time.
FYI
I would like to remind AO, that there was a pallet of bolts at the High School that we showed Shane while doing
the inventory. Are these the nuts and bolts you are referred by to?
Kevin/David, AOW is committed to providing the highest quality work and we take responsibility for any issues with
the rebar construction. I will address the concerns outlined by David with the subcontractor to ensure our deliverable matches the construction drawings. In the meantime, Kevin appears to be working on the other issues I raised yesterday regarding the grounding
and the security of the all-thread. I am attaching an image of another monopole installation we have done that includes a template plate on the top and bottom that maintains alignment of the vertical all-thread as well as the sub level concrete tension necessary
to hold the all-thread in the concrete. I will redline both the grounding implementation as well as the structural improvement required for the monopole and have this to Kevin and his team today.

Not knowing how much experience the concrete contractor has with DSA projects and Streamline Engineering plans in
placing concrete, here is one more reminder.
Have them pay attention to the Sheet S-1 Concrete Notes numbers 15, 16 & 17.
-All exposed edges are to receive 3/4” Chamfer strips
-All the Anchor Bolts are to securely positioned in place before placing concrete. Use a template. No wet setting.
Provide accurate projection with leveling nuts above the base plate for full nut threading and 3” of Non-Shrink grout between the bottom of the base plate and the footing pedestal.
-Have on hand before placing any concrete a properly sized motorized vibratory consolidator (stinger) to consolidate
the concrete fully around the reinforcement and the anchor bolts. Pushing all the air bubble out.
Just trying to be helpful now and not having to point these things out later.
FYI
All, I need the GC to redline the latest approved CD drawings with any and all field changes completed so we can
again review and determine if this is acceptable. We will have to make additional revisions to the drawings and resubmit this additional CCD to DSA for approval once we determine the extent of the changes needed. Thank you.

Additionally, there appear
to be another change to the approved plans observed.
The upper portion or the pedestal that projects up out of the footing as the part that the monopole base plate
sits 3” above, that has not been formed up yet.
Oer 7/S-2 the “Plate Detail”, that shows the pedestal portion to be 3” wider around at all sides. Further out than
the 21” base plate. Which would make the pedestal 27” x 27”.
As fabricated it appears the contractor fabricated the reinforcement steel to be 36” x 36”, not 27” x 27”.
All this may be OK? But, it is the Design Professional that decides if a Deviation from the approved plan is acceptable,
or, if adjustments are to be made.
Run all this past Kevin Sorenson.
Once the reinforcement steel deviations are resolved we wii inspect it again before you can pour.

Your Concrete contractor has installed the reinforcement steel.
Detail 6/S-2 calls for stirrups/ties to be #3 ties @ 3” O.C. vertically spaced.
There 10 of them shown on the detail.
Your contractor has used #7 rebar throughout and for the ties. They spaced the spaced them at about 7” O.C. as pictured
below.
You will need approval from the SEOR for any rebar that does not follow the detail and deviates from the DSA approved
Detail 6/S-2.
Any change will also require a CCD to DSA because it is a change.
Therefore, before you cast any concrete, and, if you plan to leave the rebar as installed; you must get the design
Professional, Kevin Sorenson approval for this change and subsequently DSA too.
According to MPR, 9/15/23, Reinforcement Inspection report there was no #3 rebar in the shipment for the Testing
Lab to sample.
Please, provide at least a 48 hour notice to myself and the Testing Lab before you pour concrete into this footing.

We have finished the rebar cage construction less the all-thread post insertion as this should be addressed before we proceed. We noticed there is no adequate grounding detail
for the equipment and monopole as the CD’s indicate that we are to be installing a bus bar on the pole and another in the IDF and that would not work as the equipment would not be protected in the event of an electrical strike. The pole and its equipment really
should be grounded in a similar manner as the other sites and that is to a grounding rod at the base of the pole.
After speaking to Joe Wargo this morning, he suggests that this has a dual ground rod application, one on each side of the pad site away from the pole exothermically welded and
contained in flex routed to emerge at the base of the pole before the slab pour. The 8’ rods would be inserted at the base of the pit. And fully inserted to natural earth and capped with the slab.
Please advise on how to proceed and suggest solutions that will be adequate to solve the issues at hand.

Good Morning Kevin,
David raised concern with the issue of there being missing detail outlining a double nut with washer application
on the all-thread stud posts that would be submerged in the concrete. I recall you and I had a phone conversation when this issue was originally brought up and your response was that provided the nuts and washers were that of the metal detail on the CD’s,
that we would be fine to add them to the application.

If what I am reading is telling us that OAW will be finishing up at Maywood Middle School and the Corning High School tomorrow,
Monday. Who will be keeping us up to date with the status of the work while commissiong at those two site is being completed? And what about the Woodson Elementary project?
Matt, Mid-Pacific Engineer (Testing Lab) and I will very much appreciate updates for any work that will be in progress at the Maywood
Middle and the Corning High School The same goes for all the remaining work to be done at the Woodson Elementary School. Be it by OAW or Nokia. We will need a contact person and their phone/email.and expect to be contacted and informed as required.
Thanks for the update. But the wiring diagram that was communicated was 0.5 not 0.4 so the deviation is due to not
using the right version.
On the RRH we will resolve it one way or another.
Pls I request AOW to delivering the scope Nokia has purchased and let us Nokia update from here onwards. Nokia
is handling the Commissioning and Acceptance of Corning next week w/o AOW. Pls take a note of it.
We are still missing the Checklist documents we requested and we hope to have them on Monday COB.
Nokia will update from here on the progress.

Just wanted to bring everyone up to date with the most recent progress and expectations going forward. AOW has completed
the week of 8/14 by bringing Maywood Middle School back to 95% completion. Per Nokia, azimuth angles were changed requiring a full dismount and reinstallation of the antennas. Cable management locks cabling into the exact length to reach the radios so any
modification of position requires the cabling to be taken loose to accommodate additional lengths or to resecure any additional slack. All of this is to say time expired Friday to make the modifications needed before we had to vacate the premise. The district
has sporting functions this weekend preventing progress and the inability to move the boom lift between schools means we will be back in Corning with a crew to finish Maywood on Monday.
Nokia has similar requests at Corning HS so we will return once Maywood is complete (approx. 3 hours of work remaining)
and make the needed adjustments to the azimuth angles, some house cleaning in the IDF and by EOD Monday, both schools will be positioned for acceptance and commissioning. Garreth from Nokia will be onsite to perform acceptance testing and confirm both schools
are online and broadcasting.
Woodson Elem is still on hold as the district has a water sewage leak leaching into the monopole pad site that has
yet to be approved for repair. In the meantime, Garreth was to bench program and test the Woodson BBU and radios however there was a switch up at some time prior to AOW involvement that pulled the 3 radios for Woodson and were used at another site. The original
boxes were still onsite and had equipment inside. However, it was not the radios so at this time I do not believe Garreth will be able to effectively set up the mock system for bench testing. Nokia is to proqure replacement radios for the future installation.
