I would have to agree mostly with Mike on what was in the Hurst transmissions.  All 440 HP engines in 1970 and 1971 had the heavy duty rods that were also used in the six pack engines in the B and E body cars, and a few Sport Fury GTs and C body police 440s.  Thus all of these engines required a specially balanced torque converter.  All of the transmissions mated to these engines also had the extra clutch disc making for a 5 clutch drum.  They also had an extra planet in the planet carriers to handle the extra a torque.  I do believe they used a 12 inch convertor though I could be wrong about that as it has been at least 10 years since I rebuilt a couple of those transmissions.  I do not recall a special ratio lever.
