Here's another thought: Was the brake fluid fresh when it was introduced into the system ? A lot of us tend to keep a half-opened bottle of brake fluid around since 'it would be a waste to discard it'. Not a good thing, since the exposed fluid will draw moisture from the air. In addition to the previously mentioned ideas, I would flush the system and use a fresh bottle of DOT 4 to refill. Castrol makes a LMA (Low Moisture Activity) variety that exceeds DOT 4 specs.
